| FAMILY FOCUS, INC. is licensed by the Minnesota Department
of Human Services under Rule Four. This authorizes FAMILY
FOCUS to license homes and place youth whose special needs
cannot be met by their own families.
We provide coordinated program services for children
and youth in placement. FAMILY FOCUS social workers are
responsible for matching youth to potential foster homes
and coordinating pre-placement visits. If the team believes
the youth is a good match with the foster family, the
social worker will facilitate this placement and schedule
the treatment plan meeting. This meeting will include
the youth and his/her family, the FAMILY FOCUS foster
parent(s), the referring worker, the FAMILY FOCUS social
worker, and other significant people involved with the
youth.
Every youth’s needs are assessed and an individualized
treatment plan is developed with the youth and treatment
team. The ongoing involvement and support of the youth’s
family is an integral part of the treatment process. The
youth and treatment team meet quarterly to review goal
progress and to re-assess the treatment plan.
FAMILY FOCUS social workers work closely with the treatment
parents to assist them in day-to-day skill building.
Treatment parents receive monies for respite and education
from the FAMILY FOCUS Administrative Rate. Respite funds
may be used in a variety of ways to provide relief for
the foster parents. Education and skill building are provided
by the FAMILY FOCUS social worker during consultation
and support groups for the parents. Families will receive
on-going training from FAMILY FOCUS to enhance their skills.
Each family will be funded to receive education through
workshops and conferences.
