| Service area and Programs |
| Family Focus serves youth and families from
all over the Minnesota area. We currently have offices located
in Austin, the Twin Cities, and Rochester. We intend to
add other offices as the need develops. We have built strong
collaborative relationships with many communities, their
respective school districts, and county agencies including
their correctional and mental health divisions. Family Focus
is a non-profit agency licensed under Rule 4; we provide
individualized care plans for youth and families. Our services
include: |
Foster Care |
| The foster care program is focused on children with special needs including severe emotional and behavioral disturbances, juvenile delinquency, teen pregnancy, and medically fragile youth. Appropriate referrals also include youth whose families are involved in concurrent planning where reunification efforts are possible. This service includes twice monthly contact from a licensed social worker that will provide supervision, crisis intervention and training to the foster parents. |
Alternative to Residential Treatment (ART) |
| ART program offers youth an intensive treatment
environment provided by experienced foster parents with
specific areas of expertise. This program may be used as
an alternative to residential placement or as an after care
program. Experienced social workers will provide weekly
case management and ongoing therapy to the youth in placement.
This program offers increased supervision, a structured
living environment and independent living skills training.
Appropriate referrals will include sex offenders, chemically
dependent youth and any other youth needing an intensive
treatment model. |
Therapeutic Support to Foster Care |
| Therapeutic support provides youth and their
foster families with education, counseling and skill development.
This program is designed to help preserve placements and
prevent disruptions. This program is paid for through medical
assistance and will be of no cost to your county. |
Professional Home Based Services |
The Professional Home Based Services Program
provides family centered home-based therapy, and skill
building/mentoring services. The goal is to prevent unnecessary
placement of youth outside the home, offer reunification
services when placement has been necessary or help prevent
disruptions while the youth is in placement by providing:
- Family assessments
- Crisis safety planning
- Intensive therapy
- Flexible hours
- Coordination with significant others
- Referral to appropriate services
All of our therapists are Masters level professionals
with experience in doing community based work. Our Life
Skills workers are Bachelor leveled practitioners who
also have experience in community based work. |

| Whole Family Placements |
| Family Focus, Inc. is very excited to announce
that we now providing whole family placement foster care
services for teen and young adult females with children.
This service is provided in the licensed foster home of
Mary Edwards, which is located in Anoka County. Mary is
currently employed with Anoka County in Adult Mental Health
Services and has expertise in crisis intervention and preventative
services, teaching parenting skills, and child protection
services. She is very knowledgeable regarding community
resources, accessing child welfare services and teaching
young females the necessary skills to fully equip them for
when they are able to move out on their own. |

Child Respite Care |
Providing short-term respite in a Rule 4 licensed foster home in order to provide relief to persons normally providing care. Serving youth experiencing severe emotional disturbances.

CADI Waiver Services |
Foster Care
CADI Waiver Foster Care is foster care for which the costs are supplemented by CADI and the Waiver.
Respite
CADI Waiver Respite offers short-term care of youth with dual or multiple diagnosis. Progress notes are completed by the foster family at the end of each visit and the TSFC program is available as a support. The $100 rate is for youth who do not require 24/7 intensive supervision and provides for a team meeting every six months. The $150 rate is for youth who require 24/7 intensive supervision, including fire setters, sexually aggressive, multiple diagnosis and/or significant health issues. Also, providers will share transportation and Family Focus will provide billing and six month reviews.

| Adoption Services |
Family Focus is a licensed agency by the Department
of Human Services to provide families with the following
supportive services during the pre- and post-adoption period.
- Recruitment and preparation of an adoptive family
for this child
- Counseling and special training of the adoptive family
- Preparation of the child for adoptive placement
- Case management and supervision of the child’s
adoptive placement
- Referral services
